How to run it

What is a decision log?
A running record of decisions: what was decided, who decided, when, the options considered and the reasoning. It stops teams reopening settled questions and helps new people understand why things are the way they are.
What should a decision log template include?
The decision in one sentence, the date, who decided, options considered, the reasoning, who's affected and a date to revisit it if needed.
How is a decision log different from a RAID log?
A RAID log tracks risks, assumptions, issues and dependencies; some versions use the D for decisions. A dedicated decision log goes deeper on the reasoning behind each choice.
