In short

A RACI chart shows who is Responsible, Accountable, Consulted and Informed for each task or decision in a project. This template builds it live at kickoff: tasks come in from everyone's phone, the room agrees exactly one Accountable per row, and rows with no owner or too many cooks are flagged and fixed in 50 minutes.

What's inside

The agenda

By the end of this meeting, every key task and decision has exactly one Accountable person, and everyone knows where they're R, C or I.

How to run it

  1. 1Define R, A, C and I in one sentence each, and repeat the rule: exactly one Accountable per row.
  2. 2Collect tasks and decisions from phones, with who people think is accountable. Different answers reveal the real gaps.
  3. 3Agree each row in turn, add the agreed line as a card and drag it to Clear, Needs an owner or Too many cooks.
  4. 4Fix the flagged rows before people leave and name who keeps the chart current. Revisit it at each phase.

Questions people ask

What is a RACI chart?
A matrix of tasks and people showing who is Responsible (does it), Accountable (owns the outcome), Consulted (gives input before) and Informed (told after) for each task.
What's the difference between a RACI chart and a RACI matrix?
Nothing; they're two names for the same thing. 'Responsibility assignment matrix' is the formal name.
What is DACI?
A decision-focused variant: Driver (runs the process), Approver (makes the call), Contributors (give input) and Informed. It suits single decisions better than ongoing tasks.
What is RAPID decision making?
Bain & Company's decision roles: Recommend, Agree, Perform, Input and Decide. Like RACI, the key is that exactly one person has the D.
