An IDT meeting, or care conference, is where a resident's interdisciplinary team meets with the resident and family to review and update the care plan. This 30-minute agenda collects the family's questions first, has each discipline report briefly, answers every question and sets goals in the resident's own words.

What's inside

By the end of this meeting, the resident and family have their questions answered and agree the care plan goals with the interdisciplinary team. The full agenda is drawn to scale above: every block has its own timer when you run it live.

How to run it

  1. 1Invite the resident and their representative, and send the join link so relatives can add questions or join from a distance.
  2. 2Collect the family's questions first, so each discipline can answer them in its report.
  3. 3Keep each report to three or four minutes and in plain language.
  4. 4Agree goals in the resident's own words and send the summary to the family afterwards.

Questions people ask

Who attends an IDT care plan meeting?
In US nursing homes, federal rules call for the attending physician, a registered nurse responsible for the resident, a nurse aide, food and nutrition staff and, as far as practicable, the resident and their representative. Therapy, activities and social services often join.
How often are care conferences held?
After admission and then after each assessment, which in US nursing homes is at least quarterly, and whenever the resident's condition changes significantly.
What should families ask at a care plan meeting?
How is my relative doing compared with last time? What are the goals? What changes are planned? Who do I call with concerns? Are advance directives up to date?
