In short

A HAZOP (hazard and operability study) is a structured team review that applies guidewords such as no, more and less to a process's design intent to find hazards and operating problems. This template runs a two-hour node session on a live worksheet board, capturing causes, consequences, safeguards and actions with owners.

The agenda

By the end of this meeting, every deviation for this node is recorded with causes, consequences and safeguards, and actions have owners and due dates.

How to run it

  1. 1Bring a multidisciplinary team: an independent chair, a scribe, and people from process, operations, instrumentation and safety.
  2. 2Set the node and design intent, then apply each guideword to each parameter in turn.
  3. 3Capture causes, consequences, safeguards and actions on the board from phones; the scribe confirms each line before moving on.
  4. 4Rank with your risk matrix, assign actions with owners and dates, and keep sessions to a few hours to stay sharp.

Questions people ask

What is a HAZOP?
A systematic hazard and operability study. A multidisciplinary team takes each part (node) of a process and applies guidewords to its parameters to find deviations, their causes and consequences, and the safeguards needed. IEC 61882 is the international standard for it.
What are HAZOP guidewords?
Short words applied to a parameter to suggest a deviation: no, more, less, as well as, part of, reverse and other than. Some studies add early, late, before and after.
What's the difference between HAZID and HAZOP?
HAZID is a broader, earlier hazard identification, often at concept stage. HAZOP is a detailed, line-by-line study once the design and drawings are well developed.
